Just had the same issue with new PS5 Slim after controller update at begining of setup process - we just manually restarted it by holding Power button and after that process succesfully finished.

Edit 4: My PS5 crashed today and asked me if I wanted to update the controller when I restarted. The update button was greyed out so I plugged the controller into the USB-C to the PS5 and updated.

The update now button is greyed out despite having my controller plugged in. I also got a new PS5 charging lead and it still doesn't work
